Special Appeals Process

Other FormsPrior approval is needed for submission of the following forms. In order to submit them securely through Self-Service Banner (SSB), our office will need to add these documents to your student record. Please email our office to discuss your individual circumstances and receive further direction.
Special Circumstances FormThis form is used to communicate changes in family income and assets, and out-of-pocket medical and dental expenses that a student would like to include in their cost of attendance. It guides students and their families through the process of explaining circumstances thoroughly and providing appropriate documentation that will be needed in addition to the form.
2024 Student Non-Filer FormIf student is not required to file a federal tax return for 2024, they must submit a Student Non-Filer Form.
2024 Parent Non-Filer FormIf parent(s) is not required to file a federal tax return for 2024, they must submit Parent Non-Filer Form.
Business Information FormFinancial aid applicants who are involved in a business, or whose families are involved in a business, may be asked to complete this form. The form will be requested at the point the application is reviewed.
Property Value Clarification FormFinancial aid applicants who own property other than their home, or whose families own property other than their own, may be asked to complete this form. The form will be requested at the point the application is reviewed.
Request to Waive Parent InformationThe Medical School OFA requests that both parents provide income and asset information on the CSS Profile. If there has been very limited or no contact with a parent for a significant period of time or perhaps, a parent’s whereabouts is unknown, the student can request that parent data be waived from the consideration in the financial aid application.
25-26 Cost of Attendance Adjustment FormPlease complete this document if you need a cost of attendance adjustment due to special circumstances.
Digital Signature InstructionsA digital ID is needed to sign financial aid documents.  Please follow the instructions to create a safe and secure digital signature.
