Institutional Aid Application |
Students applying for institutional scholarship and loans must complete the Institutional Aid Application. It is not required for students only applying for federal loans. |
CSS Profile | Students applying for institutional scholarship and loans must complete the CSS Profile with the inclusion of family information. Code: 3307 |
2022 Student Federal Income Tax Return |
Student must provide a signed copy of their tax return(s), including all schedules and W2 form(s). In the case of being married, both student and spouse must submit a signed copy of their tax return including Schedules 1, C and E and W2 form(s). Please make sure any file(s) that are uploaded are not encrypted. |
If student is not required to file a federal tax return for 2022, they must submit a Student Non-Filer Form. |

2022 Parent Federal Income Tax Returns |

Other Forms | Prior approval is needed for submission of the following forms. In order to submit them securely through Self-Service Banner (SSB), our office will need to add these documents to your student record. Please email our office to discuss your individual circumstances and receive further direction. |
This form is used to communicate changes in family income and assets, and out-of-pocket medical and dental expenses that a student would like to include in their cost of attendance. It guides students and their families through the process of explaining circumstances thoroughly and providing appropriate documentation that will be needed in addition to the form. |

2023 Student Non-Filer Form |
If student is not required to file a federal tax return for 2023, they must submit a Student Non-Filer Form. |
2023 Parent Non-Filer Form |
If parent(s) is not required to file a federal tax return for 2023, they must submit Parent Non-Filer Form. |
Financial aid applicants who are involved in a business, or whose families are involved in a business, may be asked to complete this form. The form will be requested at the point the application is reviewed. |

Property Value Clarification Form | Financial aid applicants who own property other than their home, or whose families own property other than their own, may be asked to complete this form. The form will be requested at the point the application is reviewed. |
The Medical School OFA requests that both parents provide income and asset information on the CSS Profile. If there has been very limited or no contact with a parent for a significant period of time or perhaps, a parent’s whereabouts is unknown, the student can request that parent data be waived from the consideration in the financial aid application. |
