Required FormsThese forms are required as part of the 2026-2027 financial aid application process
Free Application for Federal Student Aid (FAFSA)U.S. citizens and permanent residents will need to complete the FAFSA form, using prior-prior year income and current asset information. The Warren Alpert Medical School code is E00059.
2026-2027 Institutional Aid ApplicationStudents applying for institutional scholarship and loans must complete the Institutional Aid Application. It is not required for students only applying for federal loans.
CSS ProfileStudents applying for institutional scholarship and loans must complete the CSS Profile with the inclusion of family information. Continuing students who submitted a 2025-26 profile are exempt from this requirement. Code: 3307
2024 Student Federal Income Tax ReturnStudent must provide a signed copy of their tax return(s), including all schedules and W2 form(s). In the case of being married, both student and spouse must submit a signed copy of their tax return including Schedules 1, C and E and W2 form(s). Please make sure any file(s) that are uploaded are not encrypted.
2024 Student Non-Filer FormIf student is not required to file a federal tax return for 2025, they must submit a Student Non-Filer Form.
2024 Parent Federal Income Tax ReturnsStudent must provide a signed copy of their parents' tax return(s), including all schedules and W2 form(s). In the case of divorced parents, both parents must submit a signed copy of their tax return including Schedules 1, C and E and W2 form(s). Please make sure any file(s) that are uploaded are not encrypted.
2024 Parent Non-Filer FormIf parent(s) is not required to file a federal tax return for 2025, they must submit Parent Non-Filer Form.
2026-2027 Business Information Form To be completed if parent(s) own a business.
